GLP-1 Medicines in Ireland: What's Proven, What's Promising and What's Still Open (2026)

Every few weeks a new headline declares that weight-loss injections might protect your heart, your kidneys or your sleep — or, one day, even slow ageing. It is genuinely an exciting time for this class of medicines. It is also a confusing one, because "showed a benefit in a trial", "is approved by a regulator" and "is licensed in Ireland" are three different things. Here is the calm, evidence-based version: what large clinical trials have actually shown for GLP-1 medicines such as Mounjaro and Wegovy, what is genuinely new, what is still early research — and what honestly remains open.

Quick answer

Proven in large trials: substantial average weight loss, and — for specific medicines in specific groups of people — a lower rate of serious heart and kidney events and improved obstructive sleep apnoea. New in 2026: a daily Wegovy tablet licensed across Europe including Ireland since July, and a US approval for Mounjaro's heart-risk reduction in adults with type 2 diabetes.

Promising but not proven: retatrutide, which is not approved anywhere yet, plus early research into areas from alcohol use to ageing biology. Still open: safety over decades, why some people respond and others do not, and what happens after stopping. None of the "beyond weight" findings change what these medicines are licensed for in Ireland — weight management, prescribed only after a doctor's review. See if a doctor-led programme suits you.

First, what these medicines are

GLP-1 medicines copy the action of a gut hormone that regulates appetite and blood sugar. The result, for most people, is that food feels more satisfying and the constant background chatter about eating gets quieter. Mounjaro (tirzepatide) and Wegovy (semaglutide) are the two prescribed most often for weight management in Ireland, with Saxenda (liraglutide) as an older injectable option — and, since July 2026, a daily Wegovy tablet. We cover the mechanics in plain English in GLP-1 injections: how they work for weight loss.

What large trials have actually shown

Weight loss — the anchor use. In the STEP 1 trial, adults taking semaglutide 2.4 mg lost on average about 15% of their body weight over 68 weeks, versus about 2% on placebo. In SURMOUNT-1, tirzepatide achieved about 21% average loss over 72 weeks. Those are averages, achieved alongside structured diet and activity support — individual responses vary widely, and a meaningful minority of people lose little weight. That variation is one of the best reasons for medical supervision rather than going it alone.

Heart outcomes. The SELECT trial followed more than 17,000 adults with overweight or obesity and existing cardiovascular disease, but without diabetes. Over just over three years, a serious event — heart attack, stroke or cardiovascular death — occurred in 6.5% of the semaglutide group versus 8.0% on placebo, roughly a one-fifth lower relative risk.

On the strength of that evidence, regulators in the United States and Europe added a cardiovascular-risk indication to Wegovy's licence for that specific group. And in 2026, based on the SURPASS-CVOT trial, the US regulator similarly approved Mounjaro to reduce cardiovascular risk in adults with type 2 diabetes and established cardiovascular disease.

Kidneys. In the FLOW trial of adults with type 2 diabetes and chronic kidney disease, semaglutide reduced the risk of a composite of serious kidney and heart events by about a quarter compared with placebo. Kidney protection is an area researchers continue to study in people without diabetes.

Sleep apnoea. In the SURMOUNT-OSA trial, adults with obesity and moderate-to-severe obstructive sleep apnoea who took tirzepatide saw their breathing pauses per hour roughly halve on average over one year. The trial led to new approvals for sleep apnoea in some countries — though not as part of the weight-management licence in Ireland.

How the evidence base built up

2021STEP and SURMOUNT trials establish large average weight losses for semaglutide and tirzepatide in obesity.
2023SELECT reports fewer serious heart events with semaglutide in adults with existing cardiovascular disease.
2024FLOW reports kidney protection in type 2 diabetes; SURMOUNT-OSA reports major improvement in sleep apnoea.
2026The Wegovy tablet is licensed in Europe including Ireland (July); US approves Mounjaro heart-risk indication in type 2 diabetes.

New in 2026 — and available in Ireland now

The most practical 2026 development for people considering treatment here is the Wegovy tablet: the same active medicine as injectable Wegovy, licensed Europe-wide in July 2026 — the first daily-tablet form of a GLP-1 licensed for weight management in Ireland. Supply is still rolling out through participating pharmacies and services, and availability through any particular service depends on their clinical team's assessment, as always.

The heart-related approvals are worth understanding but easy to over-read. The 2026 Mounjaro approval described above is an American licence for a specific population — adults with type 2 diabetes and established cardiovascular disease — and European assessments run on their own timeline. None of these approvals means a weight-management prescriber in Ireland will, or should, prescribe a GLP-1 for heart, kidney or sleep reasons. Those findings matter for public health and for research; your own treatment decisions stay anchored to the licensed uses and a doctor who knows your history.

Promising — but not proven yet

Retatrutide. In a phase 2 trial, people on the highest dose lost about 24% of their body weight at 48 weeks; the phase 3 TRIUMPH-1 trial has since reported about 28% at 80 weeks, reaching roughly 30% by two years — bariatric-surgery territory. Retatrutide is not approved anywhere yet; approval applications are expected to be submitted in early 2027, and availability in Ireland is a question of future years, not months. We track it in retatrutide in Ireland: what we know.

Early research areas. Smaller and earlier studies are exploring GLP-1 medicines in alcohol use, knee osteoarthritis pain (one trial reported pain improvement), and ageing biology. A 2026 randomised trial in people with HIV suggested semaglutide slowed some measures of biological ageing — interesting, but early, in a specific patient group, and not proof for the general population.

A widely shared 2026 study on lifespan was a study in mice, which rarely translates directly to humans. If the ageing angle interests you, we examined the human evidence carefully in what the ageing research actually shows. None of these are licensed uses of any GLP-1 medicine.

Non-responders. Evidence is building that a meaningful minority of people lose little weight on GLP-1s. It is one of the more honest areas of current research — and one of the strongest arguments for a supervised programme, where response is monitored and the plan can be reviewed rather than left to drift.

What is still genuinely open

  • Safety over decades. The longest large trials run to about four years. Reassuring so far — but "so far" is the point: effects over 10 or 20 years are unknowable except by waiting. Questions researchers actively monitor include effects on the pancreas over very long horizons.
  • Muscle and bone. Rapid weight loss can include lean mass; how to protect it is well understood in principle (protein and resistance training) and covered in our guide to preserving muscle on a GLP-1. Bone health during large weight losses is still being studied — see what new research suggests.
  • What happens after stopping. In the STEP 1 extension, participants who stopped semaglutide regained about two-thirds of the weight they had lost within a year. That is not a reason to avoid treatment; it is the reason maintenance planning exists. See our weight regain and maintenance plan guide and coming off Mounjaro or Ozempic.

What all this means in Ireland

For weight management, the medicines licensed in Ireland are Mounjaro, Wegovy, Saxenda and — since July 2026 — the Wegovy tablet. Every prescription follows a doctor's assessment of your overall health, not a headline. The heart, kidney and sleep findings above belong to specific products and specific populations; they are reasons for cautious optimism about the medicine class, not promises attached to your prescription.

How a supervised programme puts the evidence to work

1

A doctor decides suitability

An Irish-registered doctor reviews your health history and confirms whether a GLP-1 medicine is appropriate for you — no medicine is issued without that review.

2

Response is monitored, not assumed

Trial averages hide wide individual variation, so progress is tracked and the plan is reviewed rather than left to drift.

3

Nutrition support protects what you keep

Dietitian input around protein and strength work supports maintaining muscle while losing weight.

4

Stopping is planned, not stumbled into

Because regain after stopping is well documented, maintenance planning is part of the programme from early on.

Weighing up treatment?

The headlines will keep coming. What stays constant is the quiet version: a short online review, a doctor who confirms what is genuinely suitable for your situation, and a programme built around how the evidence says these medicines actually behave. Start with the two-minute eligibility review.

FAQs: the evidence on GLP-1 medicines

Are GLP-1 medicines like Mounjaro and Wegovy safe long-term?

The largest trials run to about four years and are reassuring on balance, with side effects mostly gastrointestinal and mostly manageable. Safety over decades genuinely is still an open question — one reason continued medical supervision matters rather than set-and-forget prescribing.

Do weight-loss injections really protect your heart?

In the SELECT trial, adults with overweight or obesity and existing cardiovascular disease who took semaglutide had about one-fifth fewer serious heart events over three years than the placebo group, and regulators in the US and Europe added a heart-risk indication for that group. A similar approval for Mounjaro in adults with type 2 diabetes followed in the US in 2026. "Lower risk in a trial population" is not a guarantee for any individual — and heart protection is not a reason to start a weight-loss medicine by itself.

Is the Wegovy pill available in Ireland?

Yes — the daily Wegovy tablet was licensed Europe-wide in July 2026 and is available in Ireland. Whether it suits you is a doctor's decision like any other GLP-1. We cover it in detail in the Wegovy pill in Ireland.

Is retatrutide available yet?

No. Its phase 3 results (about 28% average weight loss at 80 weeks on the highest dose, roughly 30% by two years) are striking, but it is not approved anywhere in the world yet. Any service claiming to prescribe it today is one to walk away from — see our retatrutide tracker.

Does everyone lose weight on a GLP-1?

No. Averages of 15–21% in the big trials conceal a wide spread, and a meaningful minority lose little. That is normal biology, not failure — and it is exactly the kind of situation a supervised programme is built to catch and adjust.

Do GLP-1s reduce cancer risk?

Some early observational research has suggested links between GLP-1 use and lower risk of certain obesity-associated cancers, but this is not established evidence and no GLP-1 is licensed for cancer prevention. We examined one widely reported study closely in our breast-cancer-risk analysis.
